In order for this feature to work, you need to make sure you have iCloud enabled on the iPhone, and the Mac. You must also be using the exact same Apple ID on both these devices.

To double check the connection/Apple ID, open the Settings app on your iPhone and tap your ID at the top. Scroll down and your Mac should appear as a listed device.

On your Mac, open the System Preferences app and click Apple ID. At the bottom of the left column, your iPhone should be listed as a connected device.

You don’t have to enable Calls on Other Devices for every single Apple device you have. If you have more than one Mac, or an iPad, you can forgo enabling the feature for those devices. The setting for it is on your iPhone. That’s where you decide which apps can and cannot get call alerts from your iPhone.

If the feature doesn’t work, i.e., you’re unable to get call alerts, go to the Settings app on your iPhone, select Phone, and tap on the Calls on Other Devices option. Check if Wi-Fi calling is enabled. If it is, turn it off, and the alerts should appear. This isn’t a bug. Apple has designed it to work this way.
